Welcome to SIPC Participants Knowledge Space
These workstreams aim to develop the quality improvement and patient safety skills of GP practices across Scotland.
To develop their quality improvement and patient safety skills through facilitated improvement in their quality of care, in areas causing considerable morbidity and harm to patients.
Specifically Practices will:
-
Become knowledgeable and skilled in general methods for identifying, reporting and reducing harm in primary care, in order to develop their knowledge and skills in quality improvement.

Improve the reliability of care of patients with heart failure and implement.
-
reliable systems for the prescribing, management and monitoring of patient with high risk drugs including warfarin and prescribing of cytotoxic drugs such as methotrexate.
-
Develop their patient safety and improvement culture.
This work will contribute to the development of the programme and test existing tools and resources to determine their suitabiity for primary care.

SIPC 2 Project
SIPC 2 aims to develop and test small scale change packages to improve patient safety in primary care and at the interface with secondary care.
These change packages will be developed around major sources of clinical risk which present a potential for harm as patients transition across the health system:
-
Developing reliable medication reconciliation in the community.

Improving clinical communication between specialist outpatient clinics and primary care.
-
Developing safe and reliable systems for managing results.
SIPC 2 will also build expertise, experience and knowledge in patient safety within practices and NHS Boards.

Random Sampling Tool
Instructions for using the online random sampling tool:
In the 'generate' box, enter the number of patients you want in your sample.
The 'between' box should be left as 1, and the final box is the number of patients you have with the condition you are focusing on (i.e. patients on DMARDs or warfarin, patients with heart failure etc).
